Reliance Electric 61C346 four channel Analog Rail 0-10 VDC Supply: 120VAC fuse at 0.15 amp 24VDC fuse at 0.35 amp Package contains the two terminal blocks, a 45C5 cable to connect to a remote or local Remote I/O head, two clips for cable, one fuse. This has sat in a drawer in my office/lab forever. The original box is gone. Let me know if you need a manual for this.
